I need a voting poll script for PHP + MySQL

Hello everyone,
I am starting one new website and I need a voting script for it. My programming knowledge is not good enought to programme a script by my self so I am looking for something free on the net and thought maybe you all can help me.
I need:

1. A simple to use script with ability to fit it on my site layout template
2. UTF-8 encoding support becouse website is in non-English language
3. An ability to translate user interface

Would be nice if there would be, but it's not a requeiment:

• Separated page with all of the polls and results

My server is running php4 + MySQL so script shoold run on php, it can save his data in MySQL DB or text file this make no sence for me
