  Old  show spesific part of a form with a radiobutton selected?

Hello,
I want to install adsense search on my site, but it sucks for searching your own site.
So i was thinking of making a small for myself with radiobuttons,, one "site" and other "google".
So if site is selected (default) it will load the sites search and otherwise the adsense search.

But i don't know anything of javascript.
Would anyone mind pointing me in the right direction please?
I realize that this might be hard (no idea though) so if no one replys, i forgive you : D
