Looking for a programer for polling element & user login

I am looking for a programer capable of creating a poll, which reflects the user input into a data chart as well as a bar graph. I would assume php and mySQL would be all that was required for the page.

I am also looking in having a user login as well, with a simple means for the site owner to accept or reject the user, whether through an admin page or via email. The users should be able to change passwords as well as have forgotten passwords emailed.
