I do all my coding in gedit, laying out the bare bones XHTML first. Then it's CSS for general positioning of the page elements. Style tweaking (typography, coloring, etc.) comes last.
I test in Firefox first then run to browsershots.org and make sure everything is up and running in IE/Opera/Safari.

In my boxers, or sometimes with pants on. On my desktop, sometimes my notebook, other times my handheld. I use a keyboard.

Code wise I plan the XHTML out on paper in the sections first, I then create the XHTML.
I then add the CSS and style it for MOS browsers the go in and create IE stylesheets and fix them up.

I'm think he meant that he draws a rough sketch of his layout before writing the markup, not that he hand writes the HTML.

I code in e-texteditor (it's almost literally textmate for windows, lol) and I make my designs in photoshop, and yes, usually I'll draw out my designs. And no, this is not a waste of time. For me, sometimes my designs can be changed if I don't write them down because sometimes I start to change my mind on things, this keeps everything how it's supposed to be. Think rough drafts.
I have Mozilla Firefox 3, Safari, and Internet Explorer 7. I test in all three of those. Since XP has SP3, IE 6 has been obsolete (which is good to me)
