Image Upload Script

Looking to get a nice/simplistic image upload script.

Need a script, with these features:
Image upload
Hotlink for Forums
Thumbnail for Forums
Hotlink for Website
Thumbnail for Website
Direct Link

and a thumbnail of the uploaded image
the script also has to be one that has a random number at the end of the image, so the image isnt overwritten type of thing...

Side note: i have a layout already designed, if you're interested in doing this for me, ill show you exactly what im looking for...

Can I ask where you want the images stored?
In a Directory or a Database?

IE: image is stored in a directory and info about it (File url etc) is listed in a database table

Or The actual binary for the image is stored in the database ..
